The new version of application forms for notarization and legalization will be used starting from September 13, 2017.

The newly used forms Application Form of Consular Legalization of the Embassy/Consulate of the People's Republic of China and Application Form of Consular Notarization of the Embassy/Consulate of the People's Republic of China can be downloaded from the website of the Embassy of the People's Republic of China in Jamaica, by clicking Consular Service-Authentication. The old version of application forms will no longer be used after September 13, 2017.
